Service Description: Program that combines assessment, exercise and education to help reduce the risk of falls and becoming injured from a fall * participants must be able to walk 25 m and learn new information

Program includes:
  • an initial assessment provided at our clinic by a nurse practitioner and/or geriatrician and a physiotherapist
  • a two hour exercise and education sessions, twice a week for six weeks that take place at the Cawthra Community Centre located at 1399 Cawthra Rd, Mississauga
  • a follow-up visit three months after the program is completed

Fees: None - most services covered by OHIP * the City charges a fee for using their facility for the exercise sessions
Application: Physician's referral required
Eligibility - Population(s) Served: * well enough to be physically active * can walk 25 meters, with a walking aid if needed * able to learn new information * able to get to and from program * want to stay strong and independent
