Service Description:
|
Supportive housing/assisted living and psycho geriatric case management services to vulnerable and at-risk older adults * single and shared rooms * provides a range of services and supports designed to enable older adults and seniors to live safely * 24-hour on-site services are offered in response to each resident’s individual needs * a full meal program is included * offers a social activation program which includes in-house and community recreational activities * support services are available for:
- care coordination
- intensive psychogeriatric case management
- personal care and assistance with the activities of daily living
- essential housekeeping, laundry, shopping, and banking
- medication support
- escorts to appointments
- 24-hour emergency response
- crisis intervention and support
- assistance in learning or relearning basic life skills
