Service Description: Supportive housing/assisted living and psycho geriatric case management services to vulnerable and at-risk older adults * single and shared rooms * provides a range of services and supports designed to enable older adults and seniors to live safely * 24-hour on-site services are offered in response to each resident’s individual needs * a full meal program is included * offers a social activation program which includes in-house and community recreational activities * support services are available for:
  • care coordination
  • intensive psychogeriatric case management
  • personal care and assistance with the activities of daily living
  • essential housekeeping, laundry, shopping, and banking
  • medication support
  • escorts to appointments
  • 24-hour emergency response
  • crisis intervention and support
  • assistance in learning or relearning basic life skills

Application: Call * no referral required
Eligibility / Target Population: Must meet Ontario Health atHome Toronto Central criteria for Assisted Living services * adults with aging-related, mental health and addiction problems

Language Notes: Services in client's first language might be available upon request
