Service Description: Help older adults leaving hospitals to arrive home * offer clients additional support for up to 12 weeks after transitioning home from hospital

Services include:
  • assistance to book appointments and associated transportation
  • appointment reminders
  • medication reminders
  • safety checks

Application: Must have member of professional care team in hospital to complete a Home At Last referral form * visit website for referral form
Eligibility / Target Population: Adults aged 55 years or older leaving hospital emergency department or inpatients units, without support from friends or family upon discharge, and able to direct their own care
