Service Description:

Good Shepherd Non Profit Homes provides permanent supportive housing and community-based programs to individuals living with mental health and substance use- related needs. GSNPH helps people with a history of or at-risk of homelessness to maintain permanent housing and increase their quality of life. It connects individuals to resources needed to nurture and maintain social, recreational, educational, occupational and vocational activities. GSNPH uses a person-centred, evidence-based approach to care.

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
