Service Description: ACTT provides intensive, community-based support for adults (18+) in their home or another preferred community setting. The multidisciplinary team helps people work toward recovery and community integration through counselling and psychoeducation, care coordination, advocacy, and connections to healthcare and vocational supports. Services are available in North Brampton and North Mississauga for individuals with complex, ongoing needs related to severe and persistent mental illness, including those affected by homelessness, substance use, or involvement with the justice system

Multidisciplinary teams comprise of psychiatrist, psychiatric nurses, occupational therapists, social workers, peer support workers, addiction/concurrent disorder counsellors, vocational counsellors, mental health clinicians

Supports available 24 hours a day, 7 days a week
