March of Dimes Canada Independent Living 24-hour Supportive Housing Program * Non-medical personal care assistance to individuals with physical disabilities * assistance with activities of daily living * to create consumer independence and self-care in order to participate fully in community life

Funded services, no cost to client.

Self, family, or advocate, completion of Ontario March of Dimes Service Application, and Applicant Interview.
|
Eligibility / Target Population:
|
Must be at least 16 years of age or older with a permanent physical disability and/or acquired brain injury, ability to direct care, and OHIP coverage.
