Service Description: Support for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services
 
Adult Day Program - Adult day program * supervised social, recreational and therapeutic programming for seniors and adults with disabilities 
 
Assisted Living Services - 24-hour on-site personal care and support services for seniors and/or adults with disabilities. Services include:
  • medication monitoring
  • assistance with the activities of daily living
  • bathing, dressing, grooming
  • simple wound dressings
  • blood sugar and blood pressure monitoring
  • basic homemaking
  • laundry
  • light housekeeping
  • recreation
 
Case Management - Conducting an assessment, and planning and coordinating services to meet a senior's needs, goals and expected outcomes * service may be provided at a senior's home or another location * includes advocacy to assist seniors or caregivers in accessing needed services, to help them avoid falling between the cracks during service delivery
 
Eglinton Hill Centre - Offer social, educational and exercise programming to members * a range of programs and classes including exercise, dance, crafts, computers, health-promotion workshops and clinics * offer special events and outings, as well as multicultural and inter-generational activities
 
Falls Prevention and Exercise Program - Group exercise and falls prevention classes for older adults and people with mobility issues * aims to improve strength and balance to prevent injury and falls
 
Group Dining - Social activities and a hot meal * activities can include games, crafts, and physical activities * program is offered at a different location each day of the week
 
Home Help and Respite Care - Supports for the activities of daily living * includes personal care, meal preparation, laundry, and light housekeeping
 
Meals on Wheels - Delivery of hot meals * regular menu, Italian menu and Ukrainian menu * frozen meals * fruit and vegetable boxes
 
Medical Escort - Escort assistance is available for frail seniors using Toronto Ride who need more help *  this service is provided for an hourly fee * clients are encouraged to provide 2 weeks notice.
 
Transportation - Door-to-door assisted rides for seniors and/or adults with disabilities
