Service Description: Hospital-based outpatient rehabilitation for people who have had a heart transplant * care team monitors for rejection of transplant using heart biopsies and heart ultrasounds * dispenses immune medications to prevent rejection * monitors for infection, cancer, and transplant heart artery disease * services provided by cardiologists, anesthetists, cardiac surgeons, nurse practitioners, transplant nurse coordinators, social workers, psychiatrist/psychiatric nurse, physiotherapists, nutritionists, and transplant mentors

Progression and Maintenance levels of rehabilitative care for patients working to restore or maximize functionality, or prevent functional decline or injury, or to maintain functionality
