Hospital-based outpatient rehabilitation for people who have had an intestine transplant * clinic team includes a surgeon, transplant coordinator, dietitian, social worker, and psychiatrists/psychiatric nurses
Progression and Maintenance levels of rehabilitative care for patients working to restore or maximize functionality, or prevent functional decline or injury, or to maintain functionality
