Service Description: Provide primary health care, health promotion and a wide variety of community programs to the Stonegate-Parklawn Community * identify individual and community needs and respond by providing accessible and inclusive health services and health promotion programs
 
Clinical Health Services - Health services include:
  • medical services - primary health services 
  • chiropody services - foot care
  • dietitian services - nutrition education and counseling
  • asthma education - one-on-one sessions and group sessions on information to understand asthma or other breathing problems
  • diabetes education - offer nutrition education and counseling throughout the whole lifespan, from infant to older adult
 
Community Programs - Stonegate CHC provides primary health care services and health promotion and community programs in the Stonegate- Parklawn Community, identify individual and community needs and respond by providing accessible and inclusive health services and health promotion programs. The programs focus on a variety of community needs such as physical activity, healthy eating, socialization, support groups, mindfulness, chronic disease management and more. 
 
Family Programs - Support the health of individuals, families and communities
 
Fitness Program - Offer free activities to promote physical health and well being
 
Housing Help Centre - Support to the residents in the community facing difficulties in accessing housing and/or maintaining their current accommodation * advocate for residents with barriers in accessing community resources, such as financial assistance and emergency support to improve their social health
 
Seniors Programs - Variety of programs for seniors in the community
 
Women's Programs - Designed to reach out to women of all ages in the community * offer women focused services and activities, and especially welcomes women who feel disadvantaged or isolated * provide individual counselling and advocacy, and facilitates group workshops and activities on topics of special interest for women

Fees: Most medical services covered by OHIP * some medical services for uninsured clients provided for free
