Service Description: Programs and services for geriatric clients who are homebound due to complex physical, cognitive and/or psychiatric conditions * Offers three streams for clients:
  • consultation: comprehensive assessment and recommendations to referring provider 
  • shared care: shares primary care with clients' family physician 
  • primary care: assumes primary care for clients who do not have a family physician
Services provided by geriatricians, registered nurses, nurse practitioners, occupational therapist, physiotherapist, and social workers * There are three streams available:
  • Geriatric Consultation: A Comprehensive Geriatric Assessment in the home, with recommendations to the referring health care provider 
  • Shared Care: ICCT does home visits and shares the primary care with the individual’s family physician. This is usually a short-term intervention 
  • Primary Care: ICCT may assume primary care for homebound/bedbound people who cannot access to a family doctor



Referrals from a physician or nurse practitioner for this service can be initiated through our Central Navigation, which adheres to a no-wrong-door approach. This assists community agencies, primary care providers, and other healthcare professionals in navigating Ambulatory Services at Baycrest. Meetings: Assessments and visits are conducted in the clients home
Fees: There is no direct cost for this program
Application: Referral from a physician or nurse practitioner is required for a geriatric assessment and should be initiated through our Central Navigation.
Eligibility / Target Population: Adults over the age of 65 with multiple medical, functional, or psychosocial problems and unable to attend appointments onsite. Clients must reside within the catchment area of Keele Street to Yonge Street and Steeles Avenue to St. Clair Avenue
