Services and programs to meet the needs of youth and young adults with cerebral palsy or acquired brain injury as they transition from pediatric to adult rehabilitation services * provides a single point of access to specialized rehabilitation while helping patients learn how to manage their own care and navigate the adult system
Services may include:
- nutrition counselling
- spasticity management
- pain management
- speech language therapy consultation
- mobility assessment and supports consultation
- consultation around exercise programs
- short-term supportive counselling
- independent living skills development
