Service Description: Transitional housing support to older adults with serious mental health and addiction challenges who are presently in hospital (ED/ALC patients) and require transitional support services prior to achieving permanent housing in the community

Eligibility / Target Population: Older adults 60 years and older * adults with complex challenges including mental illness, addition, chronic physical health issues, social isolation, cultural dislocation, and abandonment, who need physical, social, or emotional supports
