Service Description: A clinic assessment by a geriatrician and pharmacist for seniors with age-related brittle bone disease known as osteoporosis * the physician offers strategies to slow down bone loss, prevent falls, and maintain a healthy and active lifestyle, knowledge about osteoporosis medication, calcium-rich foods and nutritional supplements

Fees: Services covered by OHIP
Application: Medical referral from the primary care practitioner, either Medical Doctor or Nurse Practitioner required
Eligibility / Target Population: Older adults and seniors with age-related brittle bones
