Service Description: Helps individuals transition back into the comfort of their own home after a brief or extended stay at a local hospital * help coordinate the support services individuals may need during this time of transition

PROGRAM HIGHLIGHTS
  • makes the transition from hospital to home easy and comfortable
  • provides a trained and caring Personal Support Worker, who will go home with the patient and stay until they are settled 
  • provides reliable and comfortable transportation
  • prepares a small meal or snack on the day the discharged patient returns home
  • helps patients feel safe, secure and cared for in their own home
  • picks up groceries, prescriptions, and medical equipment, if required
  • follow up call and assessment post discharge
Patients are responsible for the items on the following checklist
  • being organized, dressed and ready to go home by time of discharge
  • making sure that all your belongings are packed and ready to leave the hospital
  • making sure you have your house keys
  • setting up your future medical appointments
  • having enough money for groceries and for any medications that need to be picked up

Fees: Free for qualifying patients being released from participating hospitals. The Ministry of Health covers all costs for this service. There are no charges for patients participating in the program. Patients only pay for any groceries and prescriptions that are picked up.
Application: Hospital staff such as GEM Nurse, Discharge Planner, Social Workers or Nurse In-charge can make a referral
Eligibility / Target Population: Seniors 65 and up * older adults ages under 65 with related physical disabilities leaving the hospital * A client must be able to ambulate and require one-person assistance
