Service Description:
|
Provides ongoing and continuous support in the community for individuals living with severe and persistent mental illness.
Offers support including: 24-hour crisis intervention, counselling, medication education, assistance with basic necessities of daily living (housing, food, finances), access to substance use services and criminal justice system.

Application:
|
Referrals accepted from self, hospitals, physicians, and community support services and family. Referrals from St. Joseph's Health Centre Department of Psychiatry are prioritized.
Referrals are coordinated by The Access Point.
