Service Description: Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, foot care and other lifestyle choices * registered nurse, registered dietitian and foot care nurse on staff

Meetings: Monthly virtual workshops via Zoom and in-person at Centre d'acceuil héritage located at 33 Hahn Pl, Toronto, M54 4G2
