Hospital-based outpatient rehabilitation day program for older adults with multiple health problems
- health assessments
- personalized treatment plans
- health education
- referral to community services
- one-on-one and group sessions
Program runs seven to 10 weeks with visits twice a week, three hours per visit

Adults 65 years and older requiring two or more of the following services: nursing, physiotherapy, occupational therapy, social work, speech language pathology, and therapeutic recreation
