Service Description: Evidence-based group programs that empower individuals living with chronic conditions and their caregivers and foster self-management and self-efficacy skills

Evidence-based programs by South Riverdale CHC include:

Peer-led programs for people living with chronic conditions: 
  • Peer-led and mindfulness focused Art Gallery of Ontario tour
  • Peer-led art making workshops
  • Peer-led Film Talk
Programs for Health Professionals:
  • Brief Action Planning
  • Choices and Changes: Motivating Healthy Behaviour
  • Health Literacy
  • Treating Patients with C.A.R.E.


Fees: Workshops are free and open to all health and community service providers

Annually, we serve 1200 people and this work takes place in partnership with 37 organizations across the city.
