Service Description: Long-term rehabilitation program for people who have an acquired brain injury * transitions patients to a high-tolerance, low-duration rehabilitation centre where they will eventually be discharged home or to the community

Fees: Most services covered by OHIP * co-payment fee required * subsidies available to qualified applicants
Application: Medical referral required

For Health Service Providers - Submit application through the Ontario Health atHome Resource Matching and Referral program
