The GPCS is a community outreach program that provides mental health related service to individuals who are not able to access treatment at our onsite Ambulatory Mental Health Clinic and/or those who require case management.

Service Description: Treatment programs for people living with late life mental health challengesOur interprofessional services include:
  • Assessment and diagnosis 
  • Treatment including medication management and non-pharmacological approaches 
  • Case management including: advocating for clients and families 
  • Monitoring and ongoing support of clients 
  • Referrals and coordinating access to other Baycrest services and/or community programs

Referrals from a physician or nurse practitioner for this service can be initiated through our Central Navigation, which adheres to a no-wrong-door approach. This assists community agencies, primary care providers, and other healthcare professionals in navigating Ambulatory Services at Baycrest. Eligibility / Target Population: Adults over the age of 65 with a diagnosis or suspected diagnosis of depression, bipolar mood disorder, mood disorders associated with neurological conditions, mood disorders associated with bereavement, or anxiety. Patients are unable to attend onsite appointments at Baycrest or require regular case management
