Service Description:

Hospital-based outpatient program for older adults who have concerns about their physical health or memory * provides referrals to Baycrest programs and services as needed * services provided by geriatricians and other healthcare professionals
Geriatricians will be able to:
- assess your physical and mental health
- recommend a personalized treatment plan
- refer you to the appropriate specialty
clinics, programs and support services at Baycrest or elsewhere, as needed
- provide recommendations to your family
doctor to assist in coordinating your care plan

Referrals from a physician or nurse practitioner for this service can be initiated through our Central Navigation, which adheres to a no-wrong-door approach.
