Service Description: Foot care services for individuals living with Type 2 Diabetes * early intervention, regular monitoring and treatment of foot related concerns

Service include:
  • foot care education
  • foot risk assessment
  • basic foot care treatment including nail care, corns and calluses reduction
  • consultation and referrals to chiropodists in high risk situations

Fees: Free
Application: Referral from a DECNET nurse
Eligibility / Target Population: Must be a client for the diabetes program (DECNET) and referred by a DECNET nurse educator
Languages: English * French * Chinese (Cantonese) * Chinese (Mandarin)
Language Notes: Interpreters available for appointments
