Service Description:

Specialists offer short-term intensive case management, connection/ reconnection to primary care, and improved coordination of care for patients/clients with complex health needs and experiencing challenges related to the social determinants of health.

The target population is clients (18+) with complex health conditions (4+ chronic and/or high cost conditions, or fewer conditions with very significant impacts).
