Service Description: Supports for seniors 55 & older and adults with disabilities * helps people remain independent in their own homes and community for as long as possible * referral to other community services
 
Adult Day Program - Adult day program * social, recreational, and therapeutic activities for frail older adults in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
 
Client Intervention Service - Intervention and supports for people facing homelessness, critical life changes, abuse, or isolation * referrals to other community support services as necessary
 
Congregate Dining - Social activities and a hot meal * activities can include games, crafts, and physical activities
 
Foot Care - Basic foot care provided by a foot care nurse four times a month at a community clinic * foot care includes care of corns, calluses, and filing and trimming of nails
 
Griggs Manor Supportive Housing - Apartments with personal care
 
Home Help or Homemaking - Assistance with daily chores such as laundry, light housekeeping, pet care, errands, shopping, and meal preparation * service is offered weekly, bi-weekly, monthly, or seasonally
 
Home Maintenance Service - Referrals to local services such as snow removal, yard work, window washing, house cleaning, grocery shopping, and minor repairs
 
Islington Manor Supportive Housing - Apartments with personal care
 
Lerette Manor Supportive Housing - Apartments with personal care
 
Personal Care or Personal Support - Supports for the activities of daily living * includes personal care, meal preparation, laundry, and light housekeeping
 
Respite Care - Temporary relief for caregivers of people with complex medical conditions
 
Snow Removal and Grass Cutting Program - Referrals to local services such as snow removal, yard work, window washing, house cleaning, grocery shopping, and minor repairs
 
Social and Recreational Activities - Social, recreational, and educational activities * can include crafts, cards, educational lectures, and outings
 
Transportation - Non-emergency transportation to local and long-distance medical appointments and regular activities such as grocery shopping, banking, foot clinic, congregate dining and lunch programs for a fee * volunteer drivers
 
Well Elderly Program - Various activities and events are planned on a monthly basis to encourage active living, health and socialization. Activities include Day Trips, Bingos, Card Playing, Movies, Socials, etc.
