Service Description: Health and social services

Health Care Clinic -- primary health services provided by doctors and nurse practitioners * interdisciplinary care  * prenatal and postnatal care * breastfeeding clinic * sexual and reproductive health care * immunization * foot care * home visits * shelter outreach

Outreach Clinic -- walk-in, primary health care clinic for persons who are homeless or have addiction or mental health issues

Health promotion -- education and support for asthma, cancer, diabetes and hepatitis C * food and nutrition * seniors wellness and recreation * environmental health * drop-in bicycle repair clinic for low-income clients

COUNTERfit Harm Reduction Program 416-461-1925 ext 240, mobile service 416-451-1951-- distribution of harm reduction information and materials * needle exchange * counseling and case coordination * support for women working in the sex trade and using illicit drugs

Counseling and support groups -- individual counseling with a social worker * postpartum adjustment group * information and referral to other services in the community

Children and parenting -- parent and child groups for children birth-6 years * counseling for parents * healthy child screening

Physiotherapy-- prescribe personalized exercise programs designed to improve your strength, range of motion, and function
Harmony Hall Centre for Seniors - Senior's Centre - Recreation centre for older adults * supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
Diabetes Education Community Network - Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff
Family Programs - Community health and support programs for parents, grandparents, and other caregivers
Healthy Kids Community Challenge - Promote children’s health through physical activity and healthy eating
KeepSIX Supervised Consumption Service - COUNTERfit Harm Reduction program's Supervised Consumption Services
Toronto Community Hepatitis C Program (TCHCP) - Provides support , education and treatment to people living with or at risk of acquiring Hepatitis C infection
Toronto Diabetes Care Connect - Toronto Central Referral Service - Supports referrals to four different types of programs:

Fees: Most medical services covered by OHIP * some fees for other programs
Application: Call or visit * no referral required * appointments required for some programs
