Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services

Adult Day Program - Social, recreational, and therapeutic activities for Alzheimer's disease or other related dementias in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

Caregiver Support Services - Support for individuals and families caring for a relative and friend in their home, community or in a care facility * offer a range of community-based services, information, resources and referrals

Congregate Dining - Social activities and a hot meal * activities can include games, crafts and physical activities * menus match preference to nutrition recommendations * festive lunches * monthly birthday celebrations * theme days * ethnocultural meals

Home Maintenance - Snow removal & lawning * handyman is available to do small repair, painting, etc.

Home Support Programme - Assistance with housekeeping, laundry, meal preparation, shopping, banking, escorts to appointments

Meals on Wheels - Hot meals cost $8 and delivered around 10:30am-1:30pm Mon, Wed, Fri * can choose the frequency and the days for meal delivery

Telephone Reassurance Program - Regular phone calls to check on the health and safety of older adults and people with disabilities who live alone * medication reminders * heat/cold alert check in and monitoring * assessment to determine the frequency of the calls

Transportation - Transportation for medical appointments, grocery shopping, attendance at Warden Woods programs, or monthly outings * priority is given to clients requesting transportation to medical appointments

Yard Work and Snow Removal - Yard work and snow removal assistance for seniors or persons with disabilities * brokerage first initial contacts made through Warden Woods between clients and workers * clients to negotiate day and time of work and pay worker directly
