Service Description: Comprehensive assessments and care to people with neurological conditions. This includes assessment, diagnosis, and ongoing individualized treatment plans.

Individualized Treatment Plans

These plans may involve:

- Pharmacotherapy (medication)
- Non-pharmacological management (therapies or lifestyle changes)
- Counseling
- Education for patients and caregivers
- Support for family members, caregivers, and/or care partners

Conditions include:
  • Parkinson's disease
  • dementia
  • seizures
