Service Description:
|
Assists individuals age 16 and over who are having significant difficulty in day-to-day living as a result of severe and persistent mental health and substance use issues. Assists clients to meet their basic needs such as food, housing, clothing, health care, crisis management, identify and meet goals, and connect to resources and services.
