Service Description: A rapid access, intensive, short-term geriatric case management program that supports senior adults 55 and over in St. James Town, Moss Park and Regent Park, whose complex mental health illnesses or other issues (e.g. poverty, social isolation, deterioration of physical health, risk of eviction, hoarding and infestation) are impacting their ability to safely maintain their wellness and housing. Provides outreach, assessment and referrals and community-based, one-to-one, short-term intensive case management services for those seniors in the identified neighbourhoods who experience complex mental and physical health care needs.


Application: Referrals are accepted from anyone, including self-referrals. Please call for information on accessing the program.
