Service Description: Assists clients to make healthy choices and decisions * Offers educational workshops, leadership training and support to help increase self management and personal skills.

Offers the following services both on-site and off-site:

  • fall prevention
  • personal safety
  • pedestrian safety
  • nutrition awareness
  • mental health wellness
  • healthy aging
  • healthy lifestyles
  • caregivers' workshops

Six-week Self-Management Programs developed by Stanford University:

  • Diabetes Self Management
  • Chronic Conditions Self-Management Programs
