Service Description:

Offers a variety of supportive programs for older adults living in and around
the community with various levels of need
|
Adult Day Program
- Social and recreation programs, meals
provided, transportation and escort available
|
|
Case Management
- Case management services * provide practical and emotional help with navigating the health care system, accessing programs and services, supportive counseling, and crisis management.
|
|
Community Transportation
- Non-emergency, door-to-door transportation to local
medical and therapy appointments, shopping, social outings and community programs
|
|
Critical Housing Case Management
- Assistance to those who are under housed or at risk of becoming homeless to access and/or housing * case management * identifying alternative affordable housing, communicating with landlords, dealing with safety issues, avoiding eviction and solving housing-related issues * information and referral
|
|
Homemaking
- Homemaking services including laundry, vacuuming and light meal preparation
|
|
Respite Care
- One-to-one support and in-home assistance to provide caregiver relief
|
|
Security Check - Telephone Reassurance
- Volunteers provide support and reassurance through regular phone calls for companionship, to relieve isolation, provide medication reminders, check wellbeing
|
|
Telephone Reassurance
- Regular phone calls to
check on the health and safety of people who live alone * screened volunteers
listen, talk, provide important reminders and security checks to isolated
people
